I consent to allow Centerville Community Betterment, Inc. (CCBI) to conduct pre-employment background screenings as part of the hiring process. These screenings may include, but are not limited to:

  • Criminal Record Check

  • Adult and Dependent Abuse Registry Check

  • Driving Record (Motor Vehicle Report)

I understand that the results of these screenings may be used to determine my eligibility for employment and that all information will be handled confidentially and in accordance with applicable laws and regulations.

I understand that I will be required to comply with the personnel policies and rules of CCBI, and that violations of these policies may result in disciplinary action or termination of employment. I also understand that any misrepresentation or omission of facts on my application may be grounds for dismissal.
